IV administration

Magnesium replacement is a one-time dose.
Use the appropriate number of 4 g/100 mL premixed piggybacks. Infuse at 1 g per hour.

Oral administration

Elemental magnesium (magnesium oxide) or milk of magnesia may be started. Oral magnesium is poorly absorbed, and diarrhea may limit the dose. Enter a separate order for oral replacement.

Do not use for

Hemodialysis or peritoneal dialysis
Acute kidney injury
Creatinine clearance under 30 mL/min
Chronic adrenal insufficiency
Electrical burns, rhabdomyolysis, DKA, crush injury, or hypothermia
Active transfer out of the ICU or step-down unit
