Do not use for
- Hemodialysis or peritoneal dialysis
- Acute kidney injury
- Creatinine clearance under 30 mL/min
- Chronic adrenal insufficiency
- Electrical burns, rhabdomyolysis, DKA, crush injury, or hypothermia
- Active transfer out of the ICU or step-down unit
